Population Health: Diana

Like many of us, Diana has relationship issues, transportation issues, financial issues and health issues.  She also has schizophrenia.  From a health care professional's point of view it can be all too easy to consider Diana in terms of her diagnosis. But by looking at Social Determinants of Health, we understand that she is a whole person, greater than the sum of her ailments.  By keeping in mind the whole person, and all her Social Determinants of Health, health care workers are in a better position to help a person achieve optimum health. This video is one in a series of three companion videos aimed at encouraging health professionals to think more about the Social Determinants of Health, and a Population Health approach, and how these principles can be incorporated into their work with patients.
