En novembre 2014, suite à un accident à Boston, un adolescent décède mais son coeur est prélevé en vue d'une greffe. A New York, trois patients sont susceptibles de recevoir l'organe. Lorsque le coeur arrive à l'hôpital et est prêt pour l'opération, un comité a alors une heure pour déterminer qui subira la transplantation. Andre Boxer, brillant mais cynique chirurgien, Jordan Taylor, jeune et idéaliste médecin, Gilroy, docteur et bureaucrate, l'infirmière Wilkes et le prêtre Dunbar évoquent alors leurs sentiments sur chacun des trois candidats. Ils débattent afin de faire le meilleur choix pour les patients mais aussi pour l'hôpital...
