00:29back. Okay guys, Mr. Popsit here, Greg Lynch Derm, PA. And this is a common thing we see,
00:34especially with severe sun damage. Patients that worked outside a lot will sometimes get scar
00:39tissue, thickening of the skin. It's called solar elastosis and that can lead sometimes to chronic
00:46blackheads behind the ears. And I've got a couple videos where you've seen like this. We're just
00:52going to kind of open the pore a little bit, put some back pressure with my finger there,
00:55and then we'll be able to get these out for him because these can get itchy and be bothersome for
01:00sure. That's the right ear. And we have some on the left ear here as well. And you can kind of see
01:05some follicular cysts, these little pockets there. We just make a little 18 gauge usually to open that
01:11and then a little bit of back pressure and these will pop right out and go down for them so they
01:15don't itch and don't bother them as much. So thanks to him for sharing so we can see how we do these and
01:21then we'll get them all prepped and be right back. Okay guys, we have kind of prepped the area. I'm
01:25going to start up here. It's a light too bright.
01:30And we make these little openings on the pore and a little direct pressure, these pop right out.
01:36Cartilage can be a sensitive area, but he's actually a really tough guy.
01:50And it's so common for just follicular cysts here. These little pockets that get in there and they can get
02:05inflamed and then all of a sudden your ears hurt and you can't sleep. So if we can get ahead of them
02:11and do these, get these little snakes out. Call them stringers.
02:23His ears are definitely going to feel lighter.
02:25People always ask, do I come back and check? I always do.
02:39It's almost like a little sac at the base there, like that was a little follicular cyst.
02:45I'm going to come back with some forceps and we'll get that out.
02:52And down here, also, there's a couple there.

03:07Those are falling, trying to crawl away.
03:09And a lot of people will probably ask, is this my patient from four years ago that we did this on?
03:18I didn't see you four years ago, huh?
03:19No.
03:20No, exactly.
03:22I had a patient with, he was a construction worker.
03:25Very, very similar post years.
03:28And we did a clearance on his as well.
03:32It looks similar.
03:33So I know a lot of people are going to ask that.
03:34That's all I answer that question for.

03:53This one, we'll kind of push against the base there a little bit.

04:34Definitely sensitive areas, but it looks like we've cleared most of those pores out.

04:44And now he does have this.
04:48This was actually a little sac, a little follicular cyst that came out of the pore.
04:53What I'm going to do with that is we'll get a little forceps and just a little 11 blade.
05:01There's just a little fibrous connection there.
05:03It comes right out.
05:04I'm going to get that out for him.
05:07It's looking great.
05:09And now he's got two on the front of the ear here.

05:23Those are like literally pockets of keratin.
05:29One's going to be darker like a blackhead potentially.
05:34I want to go this one first here.
05:36There we go.
05:42I might need to make just a little bit bigger because this one I think it's kind of a dried
05:51out sis potentially it's pretty dry just getting a little blackhead there sorry about the pressure
06:05there it's a little clogged poor to get a baby for that one just a little cut this little sack right
06:33here it's just coming out of that pore it's usually just fibrous connected tissue there
06:51not too bad pain wise no good I just hear the flicking yeah it's kind of a flick there that's
06:58like you don't want to come out yeah it was just kind of holding on but just got to grab it and
07:03then that usually does really well a little bit more focused right there just a little stringer
07:20wasn't a big one good okay so you can see they're looking clear totally clear come down and push a
07:30little bit on the cartilage just to check those base that's looking great it's a sensitive area
07:38right up behind the ear but you can see from beginning to end there just a couple little
07:43tiny nicks on the skin you don't really see much at all and that is looking good bleeding stops within
07:48two minutes we'll go to the other side these are almost like little follicular cysts so on these ones
08:02we just make a little cut right the surface there usually that skin is so thinned out you don't really
08:09feel much of that same thing i'm going to come through let me turn just yeah just like that
08:17a little bit of pressure in these pockets
08:23keratin removal
08:30another one here i do back pressure with my finger
08:34just on the cartilage there that one rose up like a cobra huh
08:39it's gonna bite me
09:09same just a little bit of a base there
09:22tiny bit of a sack
09:25kind of comes off cuts it off about okay i'm going to come down lower right in here i'll push against
09:34it's just the bone there mastoid process good another one
09:46right under it right there
09:48very good remember he's tough not even flinching
10:04that is good a little bit more there
10:08okay now he's got a couple right in a row here i like to pull the skin so i can have some back
10:17pressure on my finger i'll pull it like this and then usually this whole pocket will start to drain
10:25so these are deep in the crease here just keratin
10:28and that's kind of you know when we're sweating and dead skin mixing together
10:38kind of up like that
10:39when we have a enlarged core and a little pocket there that sebaceous gland is just gonna keep
10:50producing sebum
10:53and then that fills up quite a bit that's what's coming out of their little streamers
10:57so that's good
10:58a little plug at the end huh
11:07i know a lot of people when they're watching them like he's not going back there is he's not going
11:10to go back to that perfect and this big one i kind of come from different angles
11:27come from the left and the right just to make sure we're getting it on that pocket
11:34and it takes quite a while usually for these to refill for the most part
11:44some people it's a couple months usually um six to twelve but i have had people like in three months
11:51they refill quickly
11:54sorry i know that's tender there got a good back pressure on it
11:57okay
12:03it's just looking up top there
12:07see if there's one last plug here
12:15a little tiny bit
12:18it's a little scar there okay so when we look here now you can kind of see
12:25just some deflated areas there's there's some scar tissue here as well
12:29um it'll be a little white like right there but overall i see one more little one and he's pretty clear
12:36so it's good to get those out um the number one thing people say about these is that they
12:42itch and sometimes it's only when you're sweating it might not be that itchy and then all of a sudden
12:46the summer comes along and they're like man these bumps are bothering me
12:51there's a little ball right there
13:08people that were rooting for the uh q-tips
13:16we got a little bit there a little ball that came out it's like amelia
13:23great
13:26okay we got them clear we'll check back on those but thanks to him for sharing it's always good i
13:31you know when i see something like this people ask you have to do that you don't have to but i think it
13:36makes them more comfortable and it's a great example to kind of show what our skin can do
13:41where these things tend to grow and just how easy it is you know in 10 or 15 minutes we can get them
13:46out so thanks to him for sharing thanks for popping back okay guys mr pops it here i always
13:50like to do a follow-up this was the large blackheads um behind the ear and you can see here this is weeks
13:56out he looks great very clear eventually they can you know kind of refill but you can see all of
14:01those pockets and clogged pores are totally clear just a little pink with post-inflammatory
14:07hyperpigment but no new blackheads there this side here he's got one trying to restart there a small
14:14one but those big ones they're looking a lot better down here it's always these pockets try to refill
14:19too so when they start refilling you know we'll give it a month or two and we'll come back and do
14:24another cleaning up for them and kind of get these unclogged because it's it's an area where we produce a lot
14:28of oil and they will refill eventually but looking way better than he was before so thanks to him
14:34for sharing and i'm glad we were able to clear that for him
