D'après une légende russe, une entité maléfique nommée la Dame de pique peut être invoquée en accomplissant un rituel ancestral devant un miroir. Selon son humeur, la créature accorde à celui qui l'appelle la réalisation d'un souhait ou lui fait subir les pires tourments jusqu'à le tuer. Pour s'amuser, Anna, Katie, Seb et Matt, quatre adolescents, décident de pratiquer ce rituel, loin d'imaginer ce qui les attend. La Dame de pique se manifeste alors, déterminée à récupérer les âmes de la petite bande d'amis...
