I work in a small clinic that was built on the grounds of the orphanage several years ago....it's really nice considering the country we are working in.
I take my own equipment to do the testing and to clean ears. There are a few that are hearing impaired and I make them new earmolds each year and repair broken hearing aids for them.
This year I brought back 5 broken aids that I will have repaired and then I will send them back to the children along with their new molds.
Going to the same orphanage each year allows me to provide fairly consistent follow up....it's far from perfect but we do pretty good considering the condition of the rest of the country.
